Our Community Transport Program provides a safe, reliable, and convenient door-to-door service, helping you maintain your independence and stay connected with your community. 

Our dedicated volunteer drivers are committed to supporting your wellbeing throughout your journey, ensuring you travel comfortably and with confidence.

Commonwealth Home Support Program (CHSP)

The Commonwealth Home Support Programme (CHSP) helps older Australians access entry-level support services so they can continue living independetly in their own homes.

The program focuses on working with you to maintain your independence, building on your strengths and abilities rather than doing tasks for you.

St John Community Care is funded to deliver Individual Social Support services across all four metropolitan funding regions - North, South, East, and West.

If you are aged 65 years or older (or 50 years or older for Aboriginal and Torres Strait Islander people), you may be eligible to access CHSP services. To receive funded servicesm you must first register with My Aged Care. St John Community Care can assist you with the registration process and help you access the support you need.

St John SA offers a range of accompanied activities to clients accessing the Commonwealth Home Support Program and Community Visitors Scheme Home Care Program.

Types of accompanied activities can include:

• Supported shopping
• Support to access Libraries/Community Centres
• Activities of Choice eg. Art or Fitness Classes
• Accompanied walks or excursions

All activities accessed through St John SA Community Care are tailored to suit individual goals and those listed above are a guide only. We will consider your individual preferences subject to volunteer availability. All clients are assessed to ensure our services are right for them.

St John SA offers a range of social outing options to clients accessing Commonwealth Home Support Program and Community Visitors Scheme Home Care Program.

Types of social outings can include:

• Visits to museums and art galleries 
• Trips to places of interest
• Movies and the arts
• Garden centres
• Walks on the beach
• Coffee and cake at a local cafe 

All activities accessed through St John SA Community Care are tailored to suit individual goals and those listed above are a guide only. We will consider your individual preferences subject to volunteer availability. All clients are assessed to ensure our services are right for them.

St John SA offers a range of in-home activities to clients accessing CHSP and ACVVS services.

Types of activities that can be undertaken together include:

• Games – such as board games and cards
• Jigsaws
• Adult colouring in
• Painting
• Music – listening or playing
• Chatting
• Craft
• Shared common knowledge
• Watching movies
• Sharing photographs
• Reading companion
• Walks in local area

All activities accessed through St John SA Community Care are tailored to suit individual goals and those listed above are a guide only. We will consider your individual preferences subject to volunteer availability. All clients are assessed to ensure our services are right for them.

The Treasured Story program creates an opportunity for people to share their life experiences with family, friends, loved ones and the community. It provides an opportunity to reflect on one’s life in a way that can bring enjoyment and peace, while at the same time leaving a legacy of living history for future generations.

This program can be accessed under your social support program or can be a stand-alone fee for service.

Sharing their life narrative gives them:

• Satisfaction
• Self esteem
• Meaning in life

Volunteer writers are individually matched according to client needs and preferences, they make weekly or fortnightly visits to the home for the duration of the program.

Our Specialised Services program offers short term intensive support in specialised activities such as:

• The Treasured Story program
• Assist with access to new technology
• Assist with access to social media
• Meaningful Moments

Clients are internally referred through the CHSP program to access programs offered through Specialised Services.
